VA OIG Review Finds Widespread Issues in Benefits System Overrides

The Department of Veterans Affairs Office of Inspector General has released a new review detailing significant problems in how the Veterans Benefits Administration handles system overrides inside the Veterans Benefits Management System for Rating. The findings point to thousands of overrides that were either unnecessary or lacked proper justification, raising concerns about payment accuracy and oversight.

According to the report, the OIG examined override activity between April 1 and September 30, 2024. During that period, an estimated 9,900 override actions were found to be unwarranted or unsupported. These actions occurred when claims processors bypassed system warnings or calculator results without providing adequate reasoning. The OIG noted that these overrides contributed to improper disability payments and unnecessary medical exams, creating both financial and administrative impacts.

The review also highlighted gaps in VBA’s oversight structure. The agency did not conduct regular quality reviews of override activity, and existing guidance did not clearly define what constitutes a valid justification. Additionally, two oversight tools within the rating system had limited functionality, reducing VBA’s ability to monitor override trends effectively.

OIG briefed VBA leadership on the findings in March 2025 and again in December 2025. While VBA acknowledged the issues, the report states that no policy changes had been implemented as of late 2025. VBA has since developed a new reporting dashboard intended to improve monitoring of overrides, though the tool remains in testing and has not yet been evaluated by OIG.

The report concludes with five recommendations aimed at strengthening guidance, improving oversight mechanisms, and enhancing system functionality. VBA agreed with all recommendations and indicated plans to address the identified weaknesses.
